Welcome to the Sleep Behavior in Typically and Atypically Developing Children survey! This survey is approved by the University of Texas Health Science Center at Houston's review board for research with human subjects.
We welcome you to complete this survey for each of your children (whether or not they have a diagnosis), but please complete it only once for each child. We do not recommend the child fill the survey out for him/herself, but he/she can certainly be consulted.
This survey is designed to be quick at easy to complete. Pages that are not relevant to you or your child will be skipped to make taking the survey fast and efficient. The survey should only take about 20-30 minutes to complete.
When answering questions, please base then on the most recent typical time of your child's life. Atypical times include those when visitors are in the home, the child is ill, the family is on vacation, etc.

We would like to emphasize that measures are in place to ensure your anonymity online and during evaluation of the survey. We regret that because of these measures it is not possible to "save" your survey and come back to it after closing it.
