Welcome to our Hilliard Lyons Grant form for Stock Market Game Teams offered by KCEE. You can receive from one to three free teams for the Stock Market Game or Take Stock in Kentucky programs in Fall 2009 or Spring 2010.

11.	Reason you need this grant?

*12.	To receive a grant, YOU MUST AGREE TO TWO of the four requirements listed below. Please check two:

	I will agree to pre/post test my class at KCEE's Online Testing Portal: http://kcee.org/testing.
	My class will take part in INVESTWRITE for the Spring session and I will submit 3 to 5 entries: http://www.investwrite.info.
	I will (or have atended) an SMG workshop or webinar during 2008 or 2009.
	I will send a thank you letter to Hilliard Lyons c/o KCEE at the end of the session:(KCEE will provide you with contact info)

BE SURE TO SUBMIT THIS APPLICATION BELOW BEFORE CLOSING.

You can THEN ENROLL your teams at www.stockmarketgame.org. If you will be using more teams than you receive free, you will be billed for the extra teams by KCEE. Please be sure to provide a Purchase Order Number on the online registration form when you register your teams at www.stockmarketgame.org. If you are using only your free teams, you will not receive an invoice.

If you will not be using your free teams, you CANNOT give them away. You will need to notify KCEE so they can be cancelled. There are a limited number of free teams to be given away each session.
